Duties and responsibilities

• Identify patients who lacked their clinical visits
•  Organizing and collaborate with CBHS community tracker
• Updating all tracking results in database cards.
• Enablishing login to track results in  database cards

• Doing weekly reviews and make sure all track results are updated

• Making sure unknown customers are excited when and 70% returns
•  Working with focal referrals, consultant and data officer
• Providing health education on the importance of considering clinics for the client
• Helping  patients on medicine and clinical tour schedules
• Recording and reporting tracking services daily, weekly and monthly

• Preparation of summary report weekly, monthly
• Participating in data management and net loss analysis
• Participating in activities that aim to increase conservation

•  Performing other duties as prescribed by the employer

QUALIFICATION

•  Certificate of Secondary Education, with a certificate or Diploma in the community work or community development or any other health related courses.

RECOMMENDED:Executive Secretary at Elizabeth Glaser pediatric AIDS Foundation

•  Must have a certificate in  primary computer application
•  One year of formal training in health or social sciences

•  One year  working in the HIV program.

•  Good communication capabilities

• Volunteers working with the ability to comply with the principles of work.

• Ability to maintain confidentiality and integrity in all areas
